AK wellbeing is committed to protecting your personal data and respecting your privacy. The privacy policy intentions are for you to know how your personal information is collected and stored via the website. All your personal information is used in accordance to Data Protection Act 1998 and in accordance with General Data Protection Regulation 2018. By using the website you agree to the collection of data in accordance with these policies. 

AK wellbeing is a private practice and has a valid ICO certificate which is what i abide by regarding handling your personal data and storage. I will only use your information to respond to you regarding any queries you may have when using my service. Your details will not be passed on to any third party without your written consent, or if required by law for any purpose. Under the Data Protection Act 2018, you may request by emailing anita@akwellbeing.org for any information I hold regarding personal details or information about you. 

Information held

Whilst using the service the information provided by you may include, but not limited to, would be 

Your details - Name, address, D.O.B, email address for correspondence for sessions, gender, age, marital status, employment status, income, disability, and any other additional information for our work together. 

Your GP's name, address and contact details, such as telephone and email. regarding any safe guarding issues.

Financial information regarding payments made for our work together.

Other methods applied when gathering data would be the information entered on the AK wellbeing website for initial contact, made by you for enquires regarding counselling.

Why is this information collected? 

To offer my clients with professional counselling services.

For the purpose of administrations and invoicing and legal obligations

To offer my clients suitable counselling session  appointment times, notification of cancellations or offer additional material relevant to counselling sessions - psychoeducation 

To seek feedback of my clients experience with the services I provided, and its effectiveness.

To keep in accordance with any safe guarding issues and procedures.

How long will your information be held for?

Clients identifiable information is held separately from any session notes. 

Clients notes/documentation is held for 3 years after which they are destroyed. All personal details are held for 7 years after which they are destroyed. 

How is data stored?

At AK wellbeing your personal information is important and how it is stored is paramount. In 

accordance to the Data Protection Act 1998(DPA) and the General Data Protection Regulation 2018(GDPR) the processing and personal data is governed by how data should be handled and stored. All paper copies are stored in a secure locked storage. Any electronic record or any  correspondence are stored and protected by a two authentication factor password and passcode  protected, on any electronic devices used.  

All measure will be taken to protect your information using protocols and procedures, as well as virus protection, security software, and encryption, I can not guarantee full security during transmission of data entered online when making enquiries or sending emails will be completely safe but i will endeavour to play my part in protecting your information, however any information you place on the website will be at your own risk, I can not offer a full guarantee of protection.

Legal use of data

Your personal data will be used when carrying out any assessment/contract and I will have your permission to do so.

If your information is required for any legal obligations - If there is mention of terrorism/drug/people trafficking in sessions.

If there is serious harm to self or others and I may need to contact any emergency service/GP as a duty of care, and under safe guarding protocol.

Ak wellbeing will adhere to Data Protection Act 1998(DPA) and General Data Protection Regulation2018(GDPR) to maintain up to date personal details, and to protect from any loss, misuse, unauthorised access and any undue disclosure to any third party..
